Maryland Health Insurance Plan is required to keep your health information private. The Notice of Privacy Practices tells you how CareFirst, the administrator for the Maryland Health Insurance Plan, uses and discloses your health information.
Please use the Privacy Request Form to send us a privacy request.
You can grant a friend or family member access to your medical records using the Privacy Authorization Form.
