How to Enroll

It is easy to apply for coverage with MHIP.

  • Step 1: Review the MHIP Application Booklet and Premium Rate Card for information on Plan costs and benefits.
  • Step 2: Print the Enrollment Application.
  • Step 3: Read the MHIP Application Booklet which explains the Plan, the eligibility criteria and the documentation that must be submitted with your application. Each section of the Enrollment Application is explained.
  • Step 4: Complete the Enrollment Application.
    Note: If the enrollment application is not complete, it will be returned to you. This will delay your enrollment in the plan.
  • Step 5: Attach the residency and eligibility documentation required for the eligibility category for which you qualify.
  • Step 6: Mail the Enrollment Application and the required documentation to:

Please note that MHIP applications may not be faxed.

Effective Dates

  • Applications received on or before the 15th of the month will be effective on the first day of the following month. For example, an application received on or before August 15th will be effective on September 1st.
  • Applications received on or after the 16th day of the month will be effective on the first day of the second month. For example, an application received on August 16th will be effective on October 1st. Note: Those individuals applying under HIPAA may qualify for other effective dates.

Current Members

Dependents may be added to or dropped from your current policy based on a qualifying event. Refer to your Certificate of Coverage for more information on qualifying events. Use the Enrollment Coverage Change Form to make changes to your policy.

Paying for your Plan

Automatic Debit lets you pay your premiums automatically. Simply authorize us to withdraw the amount due from your checking account on a monthly basis.

If you have a low or moderate income, you may qualify for reduced premiums and lower out-of-pocket costs through a program called MHIP+. Whether applying for MHIP for the first time or already enrolled, you must complete a separate MHIP+ Application.
